While both Office-Based Labs (OBL) and Ambulatory Surgery Centers (ASC) serve the medical community, they represent distinct clinical and regulatory environments. OBLs focus on catheterization and diagnostic imaging, whereas ASCs are designed for comprehensive surgical procedures and post-operative recovery.

Regulatory & Engineering

ASC construction requires a more rigorous approach. It mandates compliance with Texas HHSC Health & Safety Code Chapter 243 and Texas Administrative Code Chapter 509, followed by the stringent CMS certification process. This includes specialized electrical systems (NFPA 99), life safety systems (NFPA 101), and complex HVAC requirements.

Scope & Conversion

Conversion from an OBL to an ASC is a significant undertaking that requires meticulous planning. It involves room sizing according to FGI Guidelines, ensuring adequate recovery bays, and upgrading electrical and medical gas infrastructure to meet the higher capacity demands of a full-service surgical center.

Some practices open as an OBL and convert once volume supports it. That path works, but only if the shell decision accounts for it on day one. Ceiling height, structural capacity, corridor geometry and mechanical room space are the items that are cheap to plan for and expensive to add later. If converting is on the table, say so at the walk-through and we will build the first phase so the second one is possible.
